
Terms of Service & Business Policies
Effective date: September 3, 2026
These Terms of Service and Business Policies govern appointments, collections, screenings, verifications, mobile visits, website requests, and other services provided by Elite BioVerify Solutions, LLC (“Elite BioVerify,” “we,” “us,” or “our”). By booking, paying for, authorizing, or receiving a service, the client and each participating adult agree to these terms and to any service-specific consent, authorization, chain-of-custody form, laboratory form, employer agreement, or invoice.
1. Scope of Services
Elite BioVerify provides in-office and mobile DOT and non-DOT specimen collections; legal and informational DNA collections; DDC Peekaboo™ early gender DNA testing; FD-258 ink fingerprinting; background-check and MVR coordination; I-9 authorized-representative support; Georgia notary services; mobile phlebotomy; and employer/C/TPA compliance support. Availability depends on location, scheduling, supplies, participant eligibility, and laboratory or vendor requirements.
We do not provide STD/STI testing, general medical examinations, diagnosis, medical treatment, counseling, legal advice, immigration advice, court representation, or employment decisions. We are not a physician, attorney, court, employer, medical review officer, certifying scientist, consumer-reporting decision-maker, or government agency.
2. Identification, Consent, and Participant Responsibilities
Clients must provide accurate information, valid consent, acceptable payment, required paperwork, safe access to the collection location, and full cooperation with collection instructions. A current government-issued photo ID is required unless the applicable program expressly permits another form of identification. Names must match appointment and order information. Screenshots, photocopies, or digital images of identification are not accepted unless permitted for that specific service.
A parent, legal guardian, or properly authorized adult must consent for a minor when required and may be asked for proof of authority. We may pause, refuse, or reschedule service for missing or questionable identification, consent, payment, paperwork, safety, suspected tampering, abusive conduct, or any condition that could compromise the collection or chain of custody.
3. Appointments, Payments, Cancellations, and Fees
- Walk-ins may be accepted during posted office hours when capacity and service requirements permit. Appointments are recommended.
- An appointment is not reserved until any required deposit is paid. Deposits are applied to the service total.
- At least 24 hours’ notice is required to cancel or reschedule. With timely notice and approval, a deposit may be transferred one time to a new appointment. Late cancellations and no-shows forfeit the deposit.
- Any remaining balance is due before collection begins unless an approved written employer billing arrangement states otherwise. We accept major credit cards, debit cards, and approved electronic payments. No cash.
- A $45 mobile dispatch fee applies to most mobile appointments. After-hours, weekend, or holiday service may include an additional $55 fee. Other travel, waiting-time, rush, shipping, laboratory, vendor, administrative, specialty, recollection, cancellation, or no-show fees may apply when disclosed before confirmation or required because of client or participant actions.
- Prices shown in a booking page, written quote, invoice, or service agreement control for that service. Employer pricing is provided by request.
Deposits and payments become non-refundable after scheduling work, travel, supply preparation, laboratory or vendor orders, shipping, or administrative processing has begun, except where required by law or expressly agreed in writing.
4. Collection, Chain of Custody, and Refusal of Service
Clients and participants must follow all collection instructions. Required identity checks, observed-collection rules, temperature checks, seals, signatures, forms, and chain-of-custody procedures will be completed according to the applicable test, laboratory, employer, agency, or DOT requirements. Leaving before completion, refusing required steps, failing to provide a specimen, suspected adulteration or substitution, or disruptive conduct may be documented and reported as permitted or required.
Elite BioVerify may decline or stop a collection when conditions are unsafe, identification or consent is inadequate, instructions cannot be followed, the order is incomplete, payment is not made, or proceeding could compromise compliance or specimen integrity.
5. DNA Testing
Legal DNA testing uses identity verification, witnessed collection, documented chain of custody, and qualified laboratory processing for potential official use. Acceptance by a court, immigration authority, child-support agency, probate court, Social Security Administration, or other organization depends on that organization’s requirements and is not guaranteed by Elite BioVerify.
Informational or peace-of-mind DNA testing is for personal knowledge and generally cannot be used for court, immigration, child support, inheritance, custody, or other official purposes. Relationship probabilities and conclusions are issued by the testing laboratory. Elite BioVerify collects and coordinates samples but does not independently interpret, alter, or guarantee laboratory findings.
6. Drug Testing, DOT Services, and C/TPA Support
DOT-regulated testing is performed under applicable 49 CFR Part 40 and modal-agency requirements. Elite BioVerify acts only within its authorized service-agent role. Employers remain responsible for determining who is covered, maintaining a compliant policy, ordering the correct test, designating a DER, completing required notices and training, making employment decisions, maintaining required records, and meeting DOT and FMCSA Clearinghouse duties. A C/TPA does not replace the employer and cannot serve as the employer’s DER.
Non-DOT testing is governed by the ordering party’s written policy, applicable law, and laboratory procedures. Elite BioVerify does not decide whether an individual is hired, disciplined, removed from duty, or returned to work.
7. Background Checks, MVRs, and Fingerprinting
Background checks and motor-vehicle reports require a lawful purpose and the subject’s authorization when required. Employers and other ordering organizations are responsible for all legally required disclosures, stand-alone authorizations, certifications, notices, individualized assessments, and pre-adverse and adverse-action procedures. Elite BioVerify and its screening vendors do not make hiring or other eligibility decisions.
Reports may contain information supplied by courts, agencies, databases, motor-vehicle departments, or other third parties. Disputes concerning completeness or accuracy must follow the instructions provided with the report. FD-258 ink fingerprinting does not replace live-scan or IdentoGO processing when a requesting authority specifically requires those methods.
8. Form I-9 Support
When designated as an authorized representative, Elite BioVerify may inspect documents and complete the authorized-representative portion of Form I-9 on the employer’s behalf. The employer remains responsible for choosing the representative, determining acceptable practices, retaining the form, correcting errors, and complying with federal law. Elite BioVerify does not determine immigration status, work authorization, or eligibility for employment and does not provide immigration legal advice.
9. Laboratories, Vendors, Shipping, and Results
Some services depend on independent laboratories, medical review officers, consumer reporting agencies, couriers, technology platforms, and other vendors. By requesting the service, the client authorizes the information and specimen transfers reasonably necessary to complete it. Estimated turnaround times begin only after the appropriate laboratory or vendor receives an acceptable specimen or complete order and are not guaranteed.
Elite BioVerify is not responsible for delays, rejection, recollection, shipping interruption, vendor outage, laboratory processing, MRO review, government response time, or reporting issues outside our reasonable control. Results are released only to authorized recipients and may be withheld until required forms and payments are complete.
10. Georgia Notary Services
The fee for a Georgia notarial act is $2 per act. Mobile appointments also include the separate $45 mobile dispatch fee, and after-hours fees may apply. The signer must personally appear, present acceptable identification, demonstrate willingness and awareness, and provide a complete document with the correct notarial certificate. A notary may refuse service when legal requirements are not met.
Notaries do not select documents, draft legal language, explain legal consequences, or provide legal advice. Clients who need help choosing or completing a document should consult a licensed attorney or the requesting organization.
11. Privacy and Confidentiality
We use reasonable administrative, physical, and technical safeguards appropriate to the service. Personal information, specimens, orders, reports, and results are shared only with the client’s authorized recipients, the ordering employer or agency when applicable, service providers needed to complete the request, or as otherwise permitted or required by law.
No electronic system or delivery method can be guaranteed completely secure. Privacy obligations may also be governed by laboratory, MRO, consumer-reporting agency, employer, court, or government requirements. Elite BioVerify is not a general medical provider, and medical privacy laws apply only to the extent legally applicable to a particular service or record.
12. Results, Reports, and Client Decisions
Clients are responsible for using results and reports only for lawful, authorized purposes and for obtaining professional advice when needed. Elite BioVerify does not guarantee that a court, employer, government agency, school, licensing body, immigration authority, insurer, or other third party will accept a particular collection, report, result, fingerprint card, notarization, or verification.
Elite BioVerify does not provide medical or legal interpretation of results and does not make hiring, disciplinary, licensing, custody, immigration, insurance, court, or benefits decisions. Questions about a laboratory result should be directed through the laboratory or MRO process, and disputes concerning a consumer report should follow the reporting agency’s dispute instructions.
13. Website Information and Conflicting Documents
Website content is general information and is not medical or legal advice. Service descriptions, prices, hours, and availability may change. If these website terms conflict with a signed service agreement, employer agreement, laboratory form, chain-of-custody requirement, booking confirmation, written quote, or invoice, the more specific document controls for that service.
14. Limitation of Liability
To the fullest extent permitted by law, Elite BioVerify is not responsible for indirect, incidental, special, punitive, or consequential losses; lost income or opportunities; or decisions made by employers, agencies, courts, laboratories, vendors, or other third parties. We are not responsible for delays, failures, or inaccuracies caused by incorrect client information, participant noncompliance, shipping carriers, laboratories, MROs, screening providers, technology systems, government agencies, or events outside our reasonable control.
Nothing in these terms excludes or limits responsibility that cannot lawfully be excluded. Clients should promptly report any service concern so we can review the record and determine an appropriate response.
15. Governing Law and Severability
These terms are governed by the laws of the State of Georgia, without regard to conflict-of-law principles. Unless a written agreement states otherwise, legal proceedings concerning these terms or services must be brought in a court with proper jurisdiction in Lowndes County, Georgia. If any provision is found unenforceable, the remaining provisions remain in effect.
16. Changes to These Terms
We may update these terms as services, laws, vendor requirements, or business practices change. The effective date will be revised when changes are published. Updated terms apply to future bookings and services; a previously signed service-specific agreement remains controlling for the transaction it covers.
